Position Summary:
The Travel Home Health Nurse is responsible for providing skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans. Desired nurses will be flexible and open to travel throughout designated Houston service areas to support patient care needs and staffing coverage.

Responsibilities:
• Provide skilled nursing visits in patients’ homes
• Complete timely and accurate clinical documentation
• Coordinate patient care with physicians, therapists, and interdisciplinary teams
• Educate patients and families regarding treatment plans and disease management
• Monitor patient progress and report changes in condition
• Maintain compliance with company policies and healthcare regulations
• Travel to assigned patient locations throughout the Greater Houston area

Qualifications:

  • Valid and current LVN or LPN license in the state of Texas or compact state.
  • Home health experience preferred, but not required
  • Strong clinical assessment and communication skills
  • Reliable transportation and valid driver’s license
  • Ability to work independently and adapt to varying patient needs
  • Willingness to travel throughout assigned Houston territories
  • Experience with HCHB EMR. (preferred but not required)
  • Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
