Position Overview:

The Registered Nurse (RN) will provide safe, compassionate nursing care as a member of the profession, provider of patient-centered care, patient safety advocate and member of the health care team. The RN with a focus on Tuberculosis (TB) Care Management is required to function within the parameters of the legal scope of practice of an RN, and in accordance with the federal, state and local laws; rules and regulations; and policies, procedures and guidelines of Harris County Public Health (HCPH).

Duties and Responsibilities:

  • Works with the medical providers and the medical team to complete duties such as assisting with physical exams and medical procedures, administering/dispensing prescribed medication according to Pharmacy D regulations, and responding to emergency medical situations.

  • Assists with control of patient flow through the facility during clinic visits, including discharging patients from the clinic following the medical providers’ care plan and provides appropriate patient education.
  • Assists with patient screening (i.e., taking chest X-ray, drawing blood, testing urine, taking/recording vital signs, and performing vision and hearing assessment).
  • Receives, reviews, and prioritizes all laboratory results and ensures all abnormal laboratory values are documented in the electronic medical record (EMR) system, confirming that all abnormal laboratory values are flagged and reviewed by the medical provider for referrals or treatment as indicated.
  • Provide and explain laboratory results to patients and their families, and supplies individualized health education and counseling in a patient-centered manner. May be called upon to present group health education sessions when indicated.
  • Gives age-appropriate immunizations, stay up to date with all CDC vaccine schedule changes, and assists the nursing team with providing CDC recommended vaccinations at all times and serves as a vaccination coordinator or alternate for the Vaccine for Children’s (VFC) Program with the Texas Department of Health Services (DSHS).
  • Assists with medical inventory, including but not limited to vaccines and oral medications, ordering, and ensuring the clinic is appropriately stocked.
  • Nurse case manage tuberculosis patient on treatment  Monitor aspects of clinical care plan, including (but not limited to) pertinent medical history, signs and symptoms, medication regimens, and toxicity profiles. Responsible for coordinating all aspects of patient's TB care plan and ensuring the care plan is implemented in timely and appropriate manner.

Requirements

Education:

  • Associate's degree in nursing. 

License:

  • Current licensure as an RN in the State of Texas by the Texas Board of Registered Nurse Examiners. 

  • Basic Life Support (BLS) Certified (AHA approved).

  • Valid TX Driver's License.  

Experience:

  • One (1) year of experience as an RN in an outpatient clinical setting. 

Knowledge, Skills and Abilities:

  • Proficient in MS Office Suite (Word, Excel, PowerPoint, Outlook) and navigating Electronic Medical Record Systems. 

  • Excellent communication skills (verbal & written). 

  • Ability to establish and maintain effective working relationships and foster teamwork in a diverse environment. 
  • Knowledge of current CDC recommendations for immunizations. 
  • Ability to organize material, prioritize job tasks, make reliable judgments, and draw conclusions from verbal discussions and written data. 
  • Ability to draw conclusions from data, such as lab tests, to record this information accurately in patient’s record, and to notify the medical providers when deviations occur.

General Information

Position Type, and Typical Hours of Work:
  • This is a Full-Time | Grant-Funded position.

  • Days and hours of work are typically Monday through Friday from 8:00 am to 5:00 pm. 
  • Must be willing to flex work hours when required, including weekends. 
  • Able to travel within Harris County as needed.
Work Environment:
  • This job operates in a professional clinic environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands:
  • This role requires the ability to lift, carry, push, pull up to 50 pounds and bending & standing for periods at a time.
