Position Overview

We are seeking two experienced Registered Nurses (RNs) to provide high-quality skilled nursing care through home health field visits in the Houston area. This role is ideal for nurses with strong home health experience who are confident working independently in patients’ homes while maintaining excellent clinical judgment, patient care, and documentation standards.

Key Responsibilities

• Conduct comprehensive in-home patient assessments and skilled nursing visits.
• Develop, implement, and update individualized patient care plans.
• Provide skilled nursing services including medication administration, wound care, IV therapy, and other treatments as ordered.
• Educate patients and families on disease processes, medications, and care plans.
• Accurately document patient visits, progress, and care provided in compliance with agency policies and regulatory requirements.
• Coordinate care with physicians, therapists, and other members of the healthcare team.
• Ensure compliance with state, federal, and home health regulations.

Requirements

• Active Texas RN license in good standing.
• Proven home health nursing experience (required).
• Strong clinical assessment and patient care skills.
• Reliable transportation and willingness to travel throughout the Houston area.
• Excellent communication, documentation, and time management skills.
• Ability to work independently in a field-based environment.
