West Janisch Health Care Center is seeking a results-driven Administrator (LNFA) to steward clinical quality, operational performance, census growth, and financial outcomes—while cultivating a mission-driven, resident-first culture.

Role Summary

You’ll own the building’s overall performance: clinical outcomes, regulatory compliance, Medicare/Medicaid reimbursement and revenue cyclecensus growth & marketing, workforce engagement, and bottom-line results. You partner closely with Nursing, MDS/RAI, Therapy, Admissions/Marketing, Dietary, Social Services, Business Office, and Regional support.

Key Responsibilities:Strategy, Leadership & Culture
  • Set the vision, goals, and KPIs; model a culture of accountability, compassion, and continuous improvement.

  • Lead daily/weekly huddles (clinical, admissions, labor, revenue) and ensure cross-department execution.

Clinical & Regulatory Excellence
  • Ensure continuous survey readiness and compliance with CMS RoPs, Life Safety, OSHA, and state regulations.

  • Drive quality programs (falls, wounds, infections, readmissions) and Five-Star/QM improvement; support infection-prevention, antibiotic stewardship, and risk management.

Medicare/Medicaid Reimbursement & Revenue Cycle
  • Lead PDPM optimization through accurate assessment timing (ARDs), ICD-10 coding, clinical documentation, and care pathways with MDS/RAI, Therapy, and Nursing.

  • Oversee Triple Check and pre-bill reviews; ensure clean claims (UB-04), correct modifiers/occurrence codes, and timely submission.

  • Manage Medicaid (including Texas QIPP participation/attestation and measure performance), Medicaid eligibility, and pending conversions.

  • Direct Managed Care/Medicare Advantage authorization workflows (initial, concurrent, LOS management), and denial prevention/appeals.

  • Own revenue cycle KPIs: CMI/PDPM case mix, DSO/AR days, cash collections, write-offs/bad debt, payer mix, and ancillary capture.

Census Growth, Marketing & Outreach
  • Build and maintain relationships with hospitals/ACOs, case managers, physicians, SNFists, home health & hospice, and community partners.

  • Lead field outreach and in-hospital rounding, respond to referrals promptly, and coordinate fast, safe admissions.

  • Manage the pipeline: inquiry → admission conversion rateaverage LOS, payer mix, and occupancy targets; leverage a CRM and weekly referral source reviews.

  • Partner with clinical leadership to reduce RTH (return-to-hospital) and improve VBP-related metrics.

Operations, Finance & Labor Management
  • Own the P&L and budget; manage labor (HPPD), agency use, supplies/PPD, food/PPD, and contract services.

  • Review weekly flash and monthly financials; correct variances with clear action plans.

  • Ensure safe staffing, schedule integrity, and retention initiatives; drive engagement and reduce turnover.

People & Community
  • Recruit, develop, and evaluate department leaders; provide coaching, recognition, and corrective actions when needed.

  • Maintain strong resident/family communication; resolve grievances quickly and professionally.

Qualifications
  • Current Texas LNFA license (required).

  • 5+ years of leadership experience in SNF/Senior Care with demonstrated results in census growth and financial stewardship.

  • Deep working knowledge of Medicare (PDPM), Medicaid (including Texas QIPP), Managed Care/MA, MDS/RAI processes, ICD-10, Triple Check, and SNF revenue cycle.

  • Proven success with marketing/outreach, referral relationships, and conversion metrics.

  • Strong command of quality programs (Five-Star/QMs, infection control, readmissions/VBP) and survey readiness.

  • Financial acumen: budgets, KPIs (EBITDA/NOI, DSO, AR aging, payer mix, HPPD, PPD), and variance management.

  • Exceptional leadership, communication, problem-solving, and change-management skills.
