As a ministry of the Archdiocese of Galveston-Houston, St. Dominic Village provides a seamless continuum of care, comprised of short-term rehabilitation, skilled nursing care, assisted living, independent living, the Archbishop Joseph A. Fiorenza Priest Retirement Residence, and respite care services.
Job Overview
We are seeking a dedicated and compassionate Central Intake Nurse to join our healthcare team. The ideal candidate will have experience in long term care/skilled nursing facility.
Responsibilities and Duties
- Facilitate admissions for incoming Skilled Nursing patients (does not include direct patient care)
- Verify and enter physician orders
- Identify level of care needed
- Receive and give verbal orders accurately
- Review clinical documentation to determine Skilled Nursing placement
- Exceptional interpersonal and customer service skills
- Ability to communicate clearly with medical providers
- Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
- Manage after-hours and weekend referrals (rotating on call)
Qualifications:
- Current license as a licensed RN from the Texas State Board of Nursing
- Minimum of 3 years Skilled nursing facility experience
- Good verbal and written communication skills
- Basic computer knowledge
- Ability to effectively problem solve
- Excellent customer service skills
- Knowledge of Medicare/Medicaid Skilled Nursing placement criteria preferred
